Sinemet Plus Tablets (100 pack) combine levodopa 100mg with carbidopa 25mg. The '125mg' refers to the combined strength. In MSD's well-established Parkinson's disease treatment. Directions for Use

This is a prescription medicine. Take only as directed by your doctor. The dose is carefully tailored and adjusted over time, usually spread across the day at regular intervals. Swallow with water; taking it with a light, low-protein snack can ease nausea, though high-protein meals may reduce its effect. Take doses on time, as the benefit can wear off between doses. Never stop suddenly. This can cause a serious reaction; your doctor will adjust the dose gradually.

Tell your doctor about heart disease, glaucoma, stomach ulcers, mental health conditions or melanoma. Levodopa can cause involuntary movements, low blood pressure on standing (rise slowly), nausea and, sometimes, sudden sleepiness or impulse-control changes (gambling, overspending). Report these. Avoid high-dose vitamin B6 supplements and non-selective MAOI antidepressants. Do not stop abruptly. Use in pregnancy or breastfeeding only on specialist advice.
