Directions for Use

This is a prescription medicine used under specialist supervision only. Dosage is determined by your transplant specialist based on your individual needs and regular tacrolimus blood-level tests. Do not adjust your dose or switch tacrolimus brands without their direction, as different brands are not automatically interchangeable. Capsules are usually taken twice daily, 12 hours apart, consistently either with or without food. Never miss or double doses; missed immunosuppressant doses risk organ rejection.

Tacrolimus interacts with many medicines (certain antibiotics, antifungals, HIV medicines) and with grapefruit. Always check before adding any new medicine or supplement. It can affect kidney function, blood pressure, blood sugar and potassium, so faithful attendance at monitoring appointments is essential. It increases vulnerability to infections and, long-term, certain cancers. Use sun protection and report fevers or unusual symptoms promptly.
